Request an inspection of a completed OSD system.

To complete this form

• Tell us who is booking the inspection
• Upload a copy of your signed endorsed and approved stormwater drainage plans
• Tell us the reference number for your approved OSD designs, found on your OSD assessment approval letter
• Upload a photo of the inside of the control pit that shows the trash grate and orifice plate
• Upload a close-up photo of the orifice plate (the metal plate attachment with a hole that controls the flow of stormwater)  with a measurement of the diameter of the orifice plate.
• Upload a close up photo of the sump and weepholes
• Provide us with any other relevant information about the OSD system
• Provide your contact details

Fees

The inspection fee is $153.00.

• 1.14% – Visa or Mastercard credit card. 
• 0.88% – American Express credit card. 

There is no fee for PayPal transactions.

Once you’ve submitted this form, we will

• Send you an email and optional SMS confirming your booking request
• Request any more information, if required
• Contact you within 10 business days to confirm a time.

What happens with your information?
